Program Guidelines
- Patient’s portion of the bill is due the day of service.
- Cannot be used with another dental plan, insurance plan, or financing program.
- No refunds of plan fee will be issued at any time if the participant decides not to utilize the dental plan.
- NON-REFUNDABLE
- There will be a $50 reinstatement fee if your plan lapses.
Exclusions and Limitations
This program is a discount plan, not a dental insurance plan. It cannot be used:
- In conjunction with another dental plan, dental insurance or financing program such as CareCredit.
- For treatment which, in the sole opinion of our doctors, lies outside the realm of their capability.
- For referrals to specialists.
- For hospital charges of any kind.
- For costs of dental care covered under automobile medical or injuries under worker’s compensation.
